Fresno, CA authorities recognize that the city has an issue with substance abuse disorders. As a result, the Contracts Division of the Department of Behavioral Health (DBH), and specifically, the Substance Use Disorder (SUD) Services, have been tasked to do something about it. Their role is to develop, plan, administer, and monitor substance abuse disorder treatment and prevention services. The mission of SUD Services is to remove or reduce barriers towards seeking treatment for alcohol, drug, or other substances disorders. The focus is on both families and individuals, helping them to achieve economic independence, overall well-being, and improved health.

The Drug Medi-Cal Approach

The DBH has started the Drug Medi-Cal organized delivery system (ODS) waiver. Originally implemented in Fresno County, it has served as a role model for the rest of the state. The program was started by asking the community how they felt members of the public dealing with substance abuse disorders could better be served. Based on this, Fresno County developed a number of best practice guidelines that all rehab, detox, and other addiction treatment centers should adhere to. Their conclusion was that more services should be made available, and that access should be made easier.

Fresno County believes in the success of this program and continues to ask the community agencies, family members, clients, members of the public, and stakeholders to come together in order to share information. Furthermore, as part of the program, Fresno is committed to improving services overall. As such, all providers are now requested to receive training in:

  • CIBHS, which is a presentation on medication assisted treatment training, of particular importance to those offering medical detox and stabilization
  • ASAM-A, which focuses on the overall continuum of care
  • DMC ODS focus group information, which currently focuses on the client flow in the criminal justice system, and on adolescents.

What Is the DMC ODS Waiver Program?

DMC ODS is part of Medi-Cal that was originally started towards the end of the 1970s. Its goal was to ensure that people would be reimbursed if they required treatment for a substance abuse disorder. However, only a limited range of services were available: methadone treatment and outpatient counseling. Fresno aims to address this by widening the scope of the program, and they have been very successful in this.

Today, the program focuses on:

  • Outpatient counseling in a way that is less restrictive than it was in the past, available for at least nine hours per week
  • Detox services, both in residential hospital settings and within outpatient facilities
  • Rehab of up to 90 days of stay in an inpatient residential facility following detox
  • Various case management services, so that people can know where to receive each type of treatment, including intervention.
  • Methadone and other forms of opioid withdrawal treatments
  • Aftercare being available for all patients, regardless of the type of rehab facility they attended for treatment
  • Increase and improvement in services that are offered to young people

Through these efforts, Fresno has made a significant positive improvement in regard to this problem. It is for this reason that the city is now seen as an example of best practice.
